Information-driven care for people aged 65 and older in Sörmland
Community-based care is a transformation of the healthcare system aimed at making care more person-centred, coordinated and accessible. It is based on the individual’s needs and circumstances, with care and support provided as close to the person as possible – both geographically and in relation to their everyday life.
Through increased collaboration between healthcare providers, local authorities and other stakeholders, healthcare can become more preventative, integrated and effective, thereby contributing to better health and quality of life for the population.
The project aims to facilitate integrated, data-driven analysis across regional and local levels to provide a fact-based overview of the study population aged 65 and over living in Eskilstuna, Sörmland. In the longer term, the project will support well-founded decisions, strategic planning and more advanced academic research in the field of data-driven healthcare. The work is being carried out in accordance with a plan approved by the Swedish Ethical Review Authority.

In early autumn, a joint workshop was held as part of the Informationsdriven nära vård project. The workshop focused on launching the needs assessment and identifying key areas for future research. Participants from the region and its municipalities came together to share their perspectives and experiences. The discussions were grounded in the needs of people aged 65 and over who move between different healthcare and social care services.
The workshop began by establishing a shared understanding of the current situation and the overall needs. The discussions then focused on identifying which groups within the elderly population and which conditions are most relevant for further investigation.
The work was carried out in three stages. First, participants explored the current situation and identified key needs. Next, they defined priority groups. Finally, they jointly prioritised the areas where information-driven care could provide the greatest benefit.
Throughout the day, participants engaged in constructive discussions and shared perspectives from different parts of the healthcare and social care system. These discussions contributed to a broader understanding of today's challenges and opportunities.
The workshop provided valuable input for the continued needs assessment and for identifying priority areas within the Informationsdriven nära vård research project.

The digital inspiration workshop on Informationsdriven nära vård brought together participants from Region Sörmland and its municipalities to explore how data and information can support the development of future healthcare and welfare services.
The workshop provided valuable insights into information driven care, including experiences from Region Halland, Region Kronoberg and Ljungby kommun, as well as perspectives on future care, welfare technology, system leadership, and provided specific examples with effects of information-drivenness on wound care.
The workshop also included engaging group discussions, where participants shared experiences, challenges, and ideas for strengthening collaboration across organisations and professional areas. The discussions generated valuable perspectives on how we can facilitate information driven care through better access to, sharing, and use of data, and provided an important foundation for the continued work within the project. Looking forward, the project aims to better understand the needs of aging population with focus on individuals 65 or older and provide insights and potential improvements to meet the resource challenges.
