Information-driven care for people aged 65 and older in Sörmland

Community-based care is a transformation of the healthcare system aimed at making care more person-centred, coordinated and accessible. It is based on the individual’s needs and circumstances, with care and support provided as close to the person as possible – both geographically and in relation to their everyday life.

Through increased collaboration between healthcare providers, local authorities and other stakeholders, healthcare can become more preventative, integrated and effective, thereby contributing to better health and quality of life for the population.

The project aims to facilitate integrated, data-driven analysis across regional and local levels to provide a fact-based overview of the study population aged 65 and over living in Eskilstuna, Sörmland. In the longer term, the project will support well-founded decisions, strategic planning and more advanced academic research in the field of data-driven healthcare. The work is being carried out in accordance with a plan approved by the Swedish Ethical Review Authority.
